I saw Dr. Cook to get the MRI results on my lower back in July. We discussed my options and the preferred way to go. I told Dr. Cook that I needed some time to think about it and would let him know when I would be ready to proceed. I went home and discussed it with my wife and we decided that I should go ahead and have the surgery. I called back the same day that I met with Dr. Cook and left a message with his assistant.
